Please complete all forms in their entirety and return them to your campus nurse. Some forms require a healthcare provider signature before services or medication can be provided at school.
Medication Administration
Required for prescription and non-prescription medications that need to be administered at school.
Special Diet Request
Use this form when a student needs meal modifications because of a medical condition or dietary need.
Food Allergy & Anaphylaxis
Forms for students with severe food allergies or risk of anaphylaxis.
Respiratory Action Plan
Forms for students who need a respiratory care plan during the school day.
Seizure Care Plan
Forms for students who need a seizure management and treatment plan at school.
Diabetic Medical Management
Forms for students who need diabetes care and management support during the school day.
Private Duty Nurse Packet
Families requesting private duty nursing support should complete the required packet. All private duty nurses must register with the school district, submit to a background check, and complete fingerprint screening. Parent/Guardians must complete a private duty nursing agreement form and submit all medical orders to the campus nurs.

For elevator access or mobility-related needs, please contact your student’s campus nurse for next steps.
Before Submitting Forms
- Forms must be completed fully before they are submitted to the campus clinic.
- Forms that require a physician or healthcare provider signature must be signed before services can begin.
- Medication cannot be administered at school until the required paperwork is received and reviewed.
- New forms may be required at the beginning of each school year.
- Contact your campus nurse if you are not sure which form your student needs.
